Maintaining independence and a quality lifestyle within your own home
Community Care enables people to remain in their own homes independently for as long as possible, assisting to prevent the premature admission to residential care. Community Care support lifestyle choices by providing a diverse range of services to people in their homes.
Anglican Care's community based programs are funded throught the Commonwealth Department of Health & Ageing and include Community Aged Care Packages (CACP), for older people requiring a low level of care, or, for those requiring a higher level of support, Extended Aged Care at Home Packages (EACH). EACH packages can also cater for dementia specific care if required. Prior to being considered for one of these packages, an assessment from the Aged Care Assessment Team (ACAT) is required. All of Anglican Care's packages are tailored to suit individual needs and aim to maintain and improve the quality of life experienced by care recipients.
Community Aged Care Packages are packages of planned, flexible and coordinated services including: showering and dressing; preparing meals, housework and laundry, transport, asisstance with organising light maintenance, light housework and laundry assistance, medication supervision, shopping and social activities. Packages are designed to meet particular needs and can be adjusted as needs change.
Extended Aged Care at Home Packages provide more intensive support to stay at home, including nursing support. In addition to help with meals, housework and personal care, EACH packages generally include nursing care such as complex wound care, pain management, oxygen therapy and catheter / enema support if needed.
The fees for Community Aged Care and EACH Packages are according to the Commonwealth Guidelines and would be discussed as part of an initial meeting with one of Anglican Care's caring staff.
